The 13-year-old was using the toilets in Asda, in Ocean Road, South Shields, when he was touched in a way police have described as "inappropriate".
The teenager was unhurt, but left "traumatised" by the assault, said to have taken place at about 4.10pm last Wednesday.
Detective Inspector Peter Bent, of South Shields CID, said: "The victim of the assault is only 13 and the incident has obviously left him very traumatised.
"There would have been a lot of people going in and out of the toilets around that time and we are keen to speak to any potential witnesses."
Det Insp Bent stressed that this type of incident is rare.
The man is described as being in his late 40s to early 50s, approximately 5ft 7in tall, with short, fluffy black hair, which is going grey.
He had tanned skin, with a small goatee beard, and was wearing a blue denim jacket and an orange round neck T-shirt with a white pattern.
Detectives are also keen to trace two other men believed to have been at the urinals at the time.
One man was wearing a padded jacket, while the second man was wearing a leather jacket.
Anyone with information as to the identity of the man pictured is asked to contact South Tyneside CID on 454 7555 or (0845) 604 3043 ext 65145.
